We use your health information to provide your care, to arrange payment for that care, and to run the practice — scheduling, quality review, and training. We do not sell it, and we do not use it for marketing without asking you separately and in writing.
Your rights
You may see and receive a copy of your record, ask us to correct it, ask for a list of certain disclosures we have made, ask us to communicate with you a particular way, and ask us to restrict how we use it. You may withdraw an authorization at any time, though we cannot undo something already done in reliance on it.
Our duties
We are required by law to protect your health information, to give you this notice, and to follow the version currently in effect. If we revise it materially, we will ask you to review the new version and record that you have seen it.
